The National Diabetes Prevention Program (National DPP) is a partnership of public and private organizations working to prevent or delay type 2 diabetes. Partners make it easier for people at risk for type 2 diabetes to participate in evidence-based lifestyle change programs to reduce their risk of type 2 diabetes.

What is it?

Prevent T2 is a proven lifestyle change program designed to prevent or delay type 2 diabetes. If you are looking for resources for those who have been diagnosed with type 2 diabetes, click here. Approximately 35% of the adult population have prediabetes and 9 out of 10 don’t know they have it. Prediabetes increases your risk of type 2 diabetes, heart disease, and stroke. The good news is that type 2 diabetes is preventable through long-term structured lifestyle modification as evidenced by the National Diabetes Prevention Study. In fact, studies show that people with prediabetes who take part in a program such as Prevent T2 can decrease their risk of developing type 2 diabetes by 58% (and 71% for people over the age of 60) (CDC, 2018). Who Can Participate? 

This program is intended for adults at high risk for type 2 diabetes. Find out if you are at risk for pre-diabetes by taking the CDC Prediabetes Screening Test here.

Note, this program is not intended for those individuals diagnosed with diabetes. If you have been diagnosed with diabetes, or have had blood glucose screenings greater than 126, or A1C 6.5 or higher, please call UnityPoint Health Endocrinology, Diabetes, and Metabolism at (309) 672-4565. Please note that a referral from your primary care provider is needed for diabetes education.

By participating in this group, you will find support and accountability as you strive to make lasting lifestyle change to prevent the onset of Type 2 Diabetes. Lifestyle coaches will encourage you in creating healthy habits surrounding physical activity, mindful eating, stress management, and goal setting to ensure a healthy lifestyle change.
